A form fixes that by making the questions non-optional. Deadline, audience, format, budget, and what "done" looks like are asked once, at the point of asking, rather than reconstructed over two days of messages.

When to use a work request form

  • Design, marketing, or content request queues
  • IT and facilities tickets in teams without a service desk
  • Client work intake for agencies and freelancers
  • Internal data or reporting requests
  • Anything currently arriving as "quick favour" direct messages
Example questions

What to put on the form.

A starting point, not a rulebook: every question below is a real Questionful question type, and all of them are editable once the form is in your account.

How Questionful helps

Building it, step by step.

01

Route by request type

One dropdown at the top, one section per type. A print job and a landing page need different questions and neither requester should see the other set.

02

Make the deadline a date

A date question instead of "ASAP". Half of triage is knowing which requests genuinely collide.

03

Score the priority if it helps

Give the urgency and impact questions point values and the request arrives with a number attached, so the queue sorts itself before anyone reads it.

04

Push it into the tracker

Every submission POSTs as JSON to Zapier, Make, or your own endpoint: so a request can become a ticket, a card, or a row without being retyped.

Tips

What makes the difference.

  • Publish the form link and stop accepting requests any other way. Half measures leave you with two queues.
  • Ask what done looks like. It is the question that prevents the third revision round.
  • Keep the type dropdown short: six options at most, or people pick "other" every time.
  • Set expectations for turnaround in the confirmation message.
  • Review the request types quarterly; the "other" answers tell you what to add.
FAQ

Work Request Form questions.

Can requests be turned into tickets automatically?

Yes, via webhook. Each submission POSTs JSON with the form, the response, and every answer, so Zapier, Make, or your own service can create the ticket. Delivery happens inline with the submission and is logged on the form.

Can requesters attach files?

Yes on paid plans, up to 25 MB per file. The free plan does not accept respondent uploads.

Can different request types ask different questions?

Yes. That is the core of the template. A dropdown routes each type to its own section.

Is there a status field requesters can check?

No. Questionful captures the request; tracking the work belongs in whatever the webhook feeds. Requesters can be given an emailed copy of what they submitted.
