T-Shirt Sign Up Form
A t-shirt sign up form exists to produce one thing: a size breakdown the printer can work from. Everything else (names, delivery, colour) is bookkeeping around that number.

The reason it usually goes wrong is free text. "Medium-ish", "same as last time", and "L (or M?)" are not a purchase order. A dropdown with fixed sizes turns forty replies into a table you can order from.

Only ask for an address when you need one

Tips
What makes the difference.
- Publish a size chart link in the form intro. It removes most of the "which size am I?" replies.
- Ask for quantity as a dropdown, not a number box, nobody needs to order 400.
- Close the form on the ordering deadline and say that date in the intro.
- If you print names on the back, show that field's exact character limit.
- Order a few extra of the two most common sizes; the summary tells you which.

Can I see how many of each size were ordered?
Yes. Closed-set questions like the size dropdown are aggregated in the response summary, so the per-size count is visible without exporting anything.
Can I take payment on the form?
No. Questionful does not process payments. Most people collect the order here and send a payment link separately.
Can one person order for several people?
Yes, either by submitting the form once per person or by adding a quantity question. One submission per person keeps the size counts cleanest.
Can I close the form after the deadline?
Yes, by closing it in the editor. The link then stops accepting new orders.
