The design problem is candour. People answer honestly when the survey is genuinely anonymous, short enough to finish in a coffee break, and followed by something visible happening. Ask for a name, or ask thirty questions, and you measure who is willing to fill in surveys rather than how the team feels.

When to use a employee satisfaction survey

  • On a quarterly or half-yearly cycle, with the wording unchanged between rounds
  • Before and after a reorganisation, a move, or a change in working pattern
  • When retention is slipping and you need to know which part of the job is doing it
  • Ahead of planning, to find out where people think the load actually is
  • Alongside a manager-level survey, if you want the two views side by side
Example questions

What to put on the form.

A starting point, not a rulebook: every question below is a real Questionful question type, and all of them are editable once the form is in your account.

Question Question type
How satisfied are you with your job overall? 1 to 5, labelled at both ends, the one number you track across rounds Linear scale
How likely are you to still be here in a year? 0 to 10 Linear scale
How much do you agree with each statement? Workload is manageable, expectations are clear, I have the tools I need, my work is recognised Rating grid
How would you rate your working relationship with your manager? Keep manager questions in one section so they can be reported separately Linear scale
Which area needs the most attention? Workload, tools, communication, progression, pay, working pattern. This is the question the rest of the survey branches on Dropdown
What would make the biggest difference to that area? One open question per branch, so the answer is specific Paragraph
What should we keep doing? The question that stops the survey reading as a complaint box Paragraph
Which team are you in? Optional, and only worth asking if teams are big enough that an answer stays anonymous Dropdown
How Questionful helps

Building it, step by step.

01

Decide what anonymous means before you write a question

Leave out every identifying question, or make each one optional. Questionful never requires a respondent account, so a link survey is anonymous unless you ask who someone is. Beware the combination. Team, tenure and role together can identify a person on a small team even when no name is collected.

02

Put the whole survey on one rating grid where you can

A rating grid asks several statements against one scale in a single question, which is far quicker to answer than eight separate ones and keeps the wording identical between rounds.

03

Branch on the area, not on the score

Ask which area needs attention as a dropdown or multiple choice and route each option to its own section. Branching runs per answer choice on choice questions. A linear scale cannot route, so put the choice question next to the scale rather than trying to branch on the number.

04

Send it in paced batches and keep the same link

Email the survey to a contact list in batches, or share one link in the channel people already read. Reuse the same form each round so the response summary and the CSV export carry the history.

05

Get the results out of the tool

Export to CSV for the round-on-round comparison, and push responses to Zapier, Make, or your own endpoint if the results should land in a spreadsheet or dashboard automatically.

Tips

What makes the difference.

  • Under two minutes, every round. A long survey run once tells you less than a short one run four times.
  • Never change the wording of the tracked question. A rewrite resets the trend line.
  • Say up front what will happen with the results, and then publish what changed. Round two response rates depend entirely on round one having visibly mattered.
  • Do not filter results down to groups of two or three people. It is the fastest way to lose the anonymity you promised.
  • Ask one open question per branch rather than a general "any comments?", specific prompts get specific answers.
  • Reporting here is a per-question response summary and a CSV export. If you need cross-tabulation, weighting or significance testing, plan to do that in a spreadsheet.
FAQ

Employee Satisfaction Survey questions.

What questions should an employee satisfaction survey ask?

One overall satisfaction rating you never reword, a rating grid covering workload, clarity, tools and recognition, a manager question, one choice question naming the area that needs attention, and one open question per branch. Around eight questions total, enough to act on, short enough to finish.

Can the survey be completely anonymous?

Yes. Respondents never need an account, and the survey collects only what you ask for. Leave out name, email and team, or make them optional. Remember that a combination of optional details can still identify someone in a small team.

How often should we run it?

Quarterly or twice a year, with identical wording each time. Monthly surveys tend to produce fatigue and falling response rates; annual ones are too slow to catch anything you can still fix.

Can I compare this round against the last one?

Not inside Questionful. There is no round-on-round comparison view. Reuse the same form, export each round to CSV, and compare in a spreadsheet, or push responses to a dashboard through Zapier or Make.

Is the employee satisfaction survey template free?

This page is a guide rather than a stock template, so the CTA opens a blank form, or the AI generator on a paid plan, where a sentence gets you the first draft. The free plan covers unlimited forms and 100 responses a month with no card, which is enough for a small team; paid plans from €19 lift the cap.
