The scoring is the mechanical bit and Questionful does it for you: give the criteria point values and each evaluation arrives with a total attached, so the debrief starts from numbers rather than impressions.

When to use a interview evaluation form

  • Any role with more than one interviewer
  • Panel interviews, where independent scores matter most
  • Technical assessments and take-home reviews
  • Promotion and internal-transfer panels
  • Anywhere hiring decisions currently happen in a corridor

What to put on the form.

A starting point, not a rulebook: every question below is a real Questionful question type, and all of them are editable once the form is in your account.

How Questionful helps

Building it, step by step.

01

Fix the criteria before the first interview

Write the rating grid rows from the role requirements, once, and use them for every candidate. Criteria invented after meeting someone are how bias enters a process.

02

Score it automatically

Point values on the closed-set questions produce a total on submission, so no one is adding up scorecards by hand before a debrief.

03

Submit before you discuss

Share the link and ask for it within an hour of the interview. Scores collected after the debrief are the debrief, written down.

04

Keep the evidence

Two short open questions (strongest evidence, biggest concern) give the debrief something specific to argue about, and the export gives you a record if a decision is ever questioned.

Tips

What makes the difference.

  • Use an even number of scale points if you want interviewers off the fence.
  • Ask for evidence, not adjectives. "Handled the follow-up question badly" beats "not strong technically".
  • Never let interviewers see each other's scores before submitting.
  • Keep the same grid across candidates for one role, or the comparison is meaningless.
  • Delete the round when hiring closes. Interview notes are personal data about the candidate.
FAQ

Interview Evaluation Form questions.

Can interview scorecards be scored automatically?

Yes. Attach point values to the rating grid and the recommendation question, and each submitted evaluation arrives with a total. Written answers stay for humans to read.

Can different interview stages score different things?

Yes. Make the stage a dropdown and route each choice to a section with that stage's criteria.

Can interviewers see each other's evaluations?

Not through the form, respondents see only their own submission. Anyone with access to the form in your workspace can see all responses, which is the intended split.

Does this integrate with an ATS?

By webhook. Every evaluation POSTs as JSON to any endpoint, so an ATS that accepts webhooks, directly or via Zapier or Make, can attach it to the candidate.
